Stone Belt seeks a dedicated individual to provide program planning and support to clients served within Bloomington's Supported Living program. The Coordinator will work with interdisciplinary team members inside and outside the agency to assess, develop and implement the client's Individual Support Plans and monitor health and safety.

The SLP Coordinator will manage and maintain client benefits, including but not limited to Social Security, Medicaid, and Medicare. The SLP Coordinator oversees the operation of assigned locations and ensures compliance with state regulations; visits homes regularly and leads a team of Site Supervisors and DSPs.

    Job Requirements:

    Education:

    • A Bachelor’s degree in a related field, or a High School Diploma/G.E.D., and two years experience in services to persons with developmental disabilities. 

    Experience:

    • Management: 1 year (Preferred)
    • supporting people with disabilities: 2 years (Preferred)
